Video Summary
The lecture reviews evolving diabetes care, emphasizing individualized treatment and cardiovascular, kidney, weight, and other comorbidities—not A1c alone. Most adults have an A1c goal below 7%, while older adults or those with multiple chronic conditions may need less stringent targets to avoid hypoglycemia. Lifestyle support and modest weight loss can improve glucose control and albuminuria. Medication choice should reflect the patient’s risks. SGLT2 inhibitors are especially beneficial for heart failure and chronic kidney disease; GLP-1–based therapies generally provide greater weight loss and are particularly useful for atherosclerotic cardiovascular risk. Metformin is not contraindicated solely because ejection fraction is below 50%; it can be used in stable heart failure when kidney function permits. In older adults taking insulin, continuous glucose monitoring may reveal otherwise undetected hypoglycemia. The case concerned a 72-year-old man with diabetes, new edema, reduced ejection fraction, and severe aortic stenosis. He received a diuretic and an SGLT2 inhibitor, then underwent TAVR. His ejection fraction improved from 30–35% to 50–55%, and his A1c fell from 7.5% to 6.8%. An SGLT2 inhibitor was an appropriate treatment choice given his heart failure. The presentation also highlights evidence that bariatric surgery can produce sustained weight loss, improve A1c, reduce medication use and diabetes complications, and lower mortality. The three review questions are answered false, true, true: metformin is not barred by low ejection fraction; an SGLT2 inhibitor may be used as first-line therapy in this context; and bariatric surgery improves weight and glycemic outcomes and is associated with reduced mortality.
